Abstract
Nursing has been recognised as central in helping people to manage chronic disease and recently has been at the forefront of CDM. However, the transfer of nurse-led models developed in different contexts, particularly the US, into the UK has not replicated the patient outcomes demonstrated in the US. This paper reports a national evaluation, comparing the nurse contribution within four CDM models.
